Licensing Rule Reference 19 CSR 30-60.100 Physical Plant, Space, Supplies and Equipment 4 (C) states: The play area shall be safe for children's activities. It shall be well-maintained and free of hazards such as poisonous plants, broken glass, barbed wire, open wells, rocks and other debris, and shall have good drainage. Violation Requirements for the outdoor play area were not met as evidenced by the fence had openings that were greater than 3 1/2 inches and allow entrapment - the openings measured 7 inch gap between ground and bottom of fence. Licensing Rule Reference 19 CSR 30-60.100 Physical Plant, Space, Supplies and Equipment 4 (A) states: An outdoor play area available on or adjoining the child care property. Facilities with a capacity of more than ten (10) children shall have a fenced play area. Play areas in facilities with a capacity of ten (10) or fewer children shall be fenced when necessary for the protection of children from traffic, water or other hazards. Fences shall be at least forty-two inches (42") high. Openings in fences shall be no greater than three and one-half inches (3 1/2"). Correction Required The outdoor play area shall be on or adjoining the property and fenced as required.

Violation The walls located cafeteria was/were not in good condition as evidenced by chipping paint. Licensing Rule Reference 19 CSR 30-60.100 Physical Plant, Space, Supplies and Equipment 1 (F) states: Walls, ceilings, floors and equipment shall be finished with materials that can be cleaned easily and shall be free of splinters, cracks and chipped paint. Floor covering shall be in good condition. The facility shall be free of all lead hazards and asbestos that is friable or dangerous. Correction Required Walls, ceilings, and floors shall be made of approved materials, easily cleaned and in good condition.

Violation The requirements for handrails and guardrails were not met as evidenced by gap in stair railing on stairs leading to the new fenced in play yard.. Licensing Rule Reference 19 CSR 30-60.100 Physical Plant, Space, Supplies and Equipment 1 (B) states: Porches, decks, stairwells or other areas in approved child care space shall have protective handrails and guardrails if there is a drop-off of more than twenty-four inches (24") from which children might fall and be injured. Guardrails shall be at least thirty-six inches (36") high, and shall have bars placed at intervals of no more than three and one-half inches (3 1/2"), or have protective material to prevent a three and one-half inch (3 1/2") sphere from passing through the bars. Correction Required Handrails and guardrails shall be provided as required. Spacings at bars shall be no more than 3 1/2 inches.
