Requirements for infant/toddler sleeping equipment were not met as evidenced by a crib/playpen was not used for each infant in that a Doctor had provided a note for an infant to be sleeping in equipment recalled by the Consumer Product Safety Commission. The infant was observed to be asleep in the Fisher Price Rock n' Play which was positioned inside a pack and play.
