Violation The ceilings located the infant room and the front foyer area was/were not in good condition as evidenced by at the 5/30/2018 inspection, brown water stained tile in the back right corner (infant room) and on right side of the foyer. At the 10/3/2018 inspection, there was not evidence that this violation was corrected. Ms. Brenda stated that while she did fix both ceiling tiles, both now have problems again. Licensing Rule Reference 19 CSR 30-62.102 Personnel (1) (O) states: The licensee shall have documentation on file at the facility of current certification in age-appropriate first aid and cardiopulmonary resuscitation (CPR) training for a sufficient number of child care staff to ensure that there is one (1) caregiver at the facility for every twenty (20) children in the licensed capacity. At least one (1) caregiver with current certification in age-appropriate first aid and CPR must be on site at all times when children are present. The training shall be certified by a nationally-recognized organization, such as the American Red Cross, American Heart Association, or an equivalent certification and approved by the department. Violation The director is not routinely on duty as required, as evidenced by at the 5/30/2018 inspection the approved director had no attendance records on site to verify her work hours. At the 10/3/2018 inspection, attendance was observed but did not indicate Ms. Susan worked 40 hours each week. Additionally, Ms. Susan was not present for today's inspection and due to staff being needed for ratios it was difficult for Ms. Brenda (in charge) to complete the Director's duties while still maintain ratios and supervising children. Ms. Brenda had to continuously keep leaving the group to obtain documents in the front office and therefore it was decided that instead of continuing to look, she would stay in the classroom. Additionally, Ms. Brenda stated that today may be Susan Vogt's last day. She will contact CCFS Dixon with more information. Violation The daily activities for preschool and school-age children did not include developmentally appropriate play experience and activities as evidenced by upon arrival at 9:04am, all eleven children were observed sitting in the main preschool room watching television. Three toddlers and one 2-year-old was required to sit strapped into high chairs to watch television without any other toys. One toddler was observed pulling on another child's high chair for something to do and one 2-year-old appeared to be trying to climb out of the high chair. At 9:25am CCS Ahlemeyer asked Ms. Maria how long she was going to require the toddlers to stay seated in the seats. Ms. Maria stated that she has never required the toddlers to sit in the high chairs before today. This was the first time this was ever done. Ms. Maria proceeded to let the children out of the high chairs and let them play..
