The facility's outdoor space was not safe for children's activities as evidenced by there is a jagged stump protruding through the pea gravel, there is protruding rubber which poses a trip hazard, there is a horizontal jagged wooden piece on the playground, there is water/ice standing in one sensory table, there are 3 cans of disinfectant immediately outside the building (within the fenced playground).

In addition, the same infants had cords attached to their pacifiers. The pacifiers were in the playpens as they were sleeping.
