Soft objects, loose bedding or any object that can increase risk of entrapment, suffocation or strangulation was observed as evidenced by a mattress under a 5 month old child in a crib was propped up using a boppy pillow. Ms. Linda stated that the parent had requested that they prop the mattress. Any alternative sleeping positions for a child under 12 months requires a doctor's note and instruction.

This letter will include the date of the Family Care Safety Registry and the date of the criminal background screening, was/were employed/present prior to the provider requesting the results of a criminal background check.
