The requirements for documentation of 12 clock hours of annual training were not met as evidenced by there were insufficient training documents on file. The staff listed needs the following information: The 2018 toolbox report has documentation of 5 clock hours for 2018. There are certificates onsite showing an additional 5 hours (1 hour is from 2019 to be rolled back to 2018) Sharon needs an additional 2 clock hours to apply to 2018 and then begin to take 12 clock hours to apply for 2019 training requirements.
