The facility's outdoor space was not safe for children's activities as evidenced by the presence of exposed footings on climbing equipment and black tarp under resilient material that was not uniformly spread.

19 CSR 30-60.100 Physical Plant, Space, Supplies and Equipment 4 (C) states: The play area shall be safe for children's activities. It shall be well-maintained and free of hazards such as poisonous plants, broken glass, barbed wire, open wells, rocks and other debris, and shall have good drainage.

The playground had not been in use for over 3 days because of heavy rainfall and extreme high temps with high heat index. Before the children go out to the playground it is checked because the high rains moves the mulch. We keep a rake on the property to fix any issues before the children are allowed on the playground.

Hazardous items as follows were accessible to children: plastic bags, trash bags, and cooking knives. The item(s) was/were located throughout the facility and in the kitchen drawers.

Kitchen utensils and kitchen cutlery (knives) are kept inside the kitchen drawers. There are two doors to the kitchen that remain closed when in use and when not in use. The kitchen is the only place these items are kept.
Violation
Children were not restricted to approved space as evidenced by the door in the back area of the middle room was unlocked. This door leads to the bell pull area and sanctuary area.
