Soft objects, loose bedding or any object that can increase risk of entrapment, suffocation or strangulation was observed as evidenced by an infant was observed sleeping in a swaddle/sleep sack. The infant was on his stomach and facing into the corner of the pack n play. The infants arms were enclosed in to swaddling of the sleep sack. The use of this sleep sack in this situation, in which the infant can roll into any position can cause a risk of entrapment and suffocation.

G states: Cribs, portable cribs and playpens shall be free of soft objects, loose bedding or any object that can increase the risk of entrapment, suffocation or strangulation. Examples include bumper pads, pillows, quilts, comforters, sleep positioning devices, sheepskins, blankets, flat sheets, cloth diapers, bibs, pillow-like toys, wedges, infant positioners, special mattresses, special sleep surfaces, and other similar items.

A child who showed evidence of wanting to feed him/herself was not encouraged and permitted to do so as evidenced by children over the age of 12 months were being spoon fed the main part of the lunch meal. CCFS Dixon questioned why the children were not feeding themselves and the staff stated it was "messy". Children should be encouraged to begin to feed themselves all parts of the meal when they are deveopmnetally ready. Additonally, children of this age could be provided utensils to practice self feeding. .
