The provider did not meet requirements for a criminal background check. Results were missing for: Provider Deborah Tiek, household members Frederick Tiek and Brandon Tiek, and approved assistant Chelsey Wolfe. Provider showed receipts for finger prints for all child care staff and is awaiting the results of the SCCR.

19 CSR 30-61.055 License Renewal 3 states: Child care staff members shall have qualifying background screening results on file as required by 19 CSR 30-63.020 General Requirements, prior to renewal of the license.

Hazardous items as follows were accessible to children: knives, Boudreaux's butt paste, Desitin, A&D ointment, Aquaphor, Febreeze air freshener. The item(s) was/were located in the kitchen, front room, and hallway bathroom. The diaper creams and Febreeze were placed out of reach of children during the inspection.

Children were not restricted to approved space as evidenced by In the hallway there is a door leading to the basement with a set of stairs. The door was closed but accessible to children. Provider Deborah Tiek stated that no one had ever said anything about it before. She stated that she would put a lock or latch on the door.
