Verification of safe sleep training was not documented and maintained as evidenced by there was no safe sleep certificate at the facility available for review for Sandy Berry. Sandy Berry took safe sleep 4-1-2017 and the safe sleep certificate was available for review at the 9-19-2019 compliance monitoring inspection. During the current inspection Sandy Berry was unable to find it, and the training is not documented for verification in toolbox.
