The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence had openings that were between 3.5 and 9 inches and allow entrapment - the openings measured 5". In addition, the railings are pulling away from the fencing. In addition, there are sharp screws and nails exposed to children.

There is unfinished sheetrock exposed in these two rooms. On October 26, 2020, CCFS Tuschhoff observed an additional wall trim outside of the playroom pulling away from the wall. There was a bucket of cigarette butts, cigarette butts on the ground, a lighter and a pack of marlboro cigarettes in a chair,and a pack of marboro cigarettes on a stand on the facility's porch. In addition, Tabatha Thurman stated she smokes on the front porch while the children are inside the facility. A child in care also told CCFS Tuschhoff that Tabatha Thurman and Assistant Chantel Bosman smokes on the front porch while kids (including the babies) are inside the facility.
