The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by the door to the garage was open, and in the garage, there was paint/primer that the children could have access to with the door remaining open Additionally, there was a plastic bottle in the back yard on the ground near the back of the fence which was trash.

The following staff need documentation of a facility orientation: the new employee orientation form was available in the employee file for Barbara Whitfield, and both Jennette Williams, owner, and Barbara Whitfield stated that the new employee orientation had been completed. Despite this statement, the new employee orientation form was missing several initials in designated spots. Jennette Williams and Barbara Whitfield ensured that the form was accurately completed during the course of this inspection to reflect that the new employee orientation had been completed as stated.
