2 15 MIXED Karen Dudley and Summer Witcher with 15 children (4 children age 2 years, 11 children ages 3-5 years) in the preschool room. Ratio was broken during the transition from lunch to nap. Summer Witcher remained in the classroom with 13 children, while the other staff member, Karen Dudley, took two children (1 child age 2 and 1 child age 3 years) to the bathroom for a diaper change and bathroom use. Chelsea Negrete remained in the hallway supervising the bathroom, but did not step into the classroom to cover ratio while Ms. Dudley was across the hall in the bathroom.
