The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence had openings that were between 3.5 and 9 inches and allow entrapment - the openings measured 3.5 inches and was located at the bottom of the fence. This section of the fence was located near the pirate ship.

The premises were not safe and suitable as evidenced by CCFS arrived to the facility at 9:08 A.M. CCFS observed an infant in a car seat. The infant was three months old. The child had a blanket placed over their torso area and legs. The child had a small piece of fabric that appeared to be a burb cloth or hand towel placed around the child's face. The provider stated that the fabric was placed around the child's face to hold in the child's pacifier. It was discussed with the provider that the fabric must be immediately removed and the safety concerns. The provider removed the items out of the car seat. A child grabbed the blanket and was covering the child's face with the blanket. The provider removed the blanket. CCFS observed other children rocking the car seat with the infant in it. When asked, the provider stated that the child was not recently dropped off. According to the sign in sheet, the child was dropped off at 7:20 A.M. The provider stated that the child usually stays in the car seat so the child can always be near the provider. The provider stated that the parents are aware and okay with the child being in the car seat. CCFS discussed with the provider that is is not developmentally appropriate for the child to be in a car seat. CCFS recommended that the child was placed in a bouncer or swing. The provider does not have a bouncer or swing. It was discussed with the provider that she would need to find an alternative and appropriate method. The child was in the car seat for approximately 25 minutes. Once removed from the car seat, the provider held the child. On 7-1-21 a re-inspection was conducted. At arrival, the child was in a bouncer that resembled a car seat. The provider had the original box that stated the seat was a Good Vibes Infant Carrier. The carrier has soothing vibrations, gentle bounce and three positions for carrying ,feeding or soothing motion. The seat also had a waist strap and handle for carrying. The child had a blanket that was placed over the child's legs. This blanket was removed. The provider stated the child is placed in the bouncer for a brief period of time in the morning. It was discussed that the child should not be placed in the bouncer for an extended amount of time.
