The requirements for protective outlet covers or twist-lock outlets were not met as evidenced by an electrical plate was broken in that the bottom left corner of the plate was chipped and had sharp edges. This was located on the green wall in the preschool/school age area.

This was located on the I/T play area along the back side of the fence.

The gate was not secure at the bottom. This was located on the far right side of the play area.
