The facility's outdoor space was not safe for children's activities as evidenced by there are exposed metal strips with barb-like protrusions between the concrete walkways and the grass carpeted areas in the large rear (school age) play area. Additionally, the grass carpet in the back corner play area under the blue awning is not in good condition as it is tearing and splitting creating trip hazards.
