The facility's outdoor space was not safe for children's activities as evidenced by there being several places throughout the preschool outdoor play area that contains tree roots that are sticking up out of the ground, and a tree stump. The roots and tree stump increase the risk for child(ren) to potentially fall and or cause injury .
