When CI Wilmesher and CI Fyfe arrived during the inspection on December 16, 2021, Lakeeta Butler was not present. Other staff members, Tiana Lambert-Evil, Taylor Eberhardt, and Allye Shirley, all identified Thekra Campbell as the director of the facility. Lakeeta Butler arrived at approximately 1:00 p.m., and admitted that she has no attendance records for herself, and she has not been working 40 hours per week for the facility.

CI Wilmesher observed at 11:48 a.m. that the same child was still in the same timeout spot. CI Wilmesher addressed the concern with caregiver, Tiana Lambert-Evil, who stated the child was placed in timeout just before CI Wilmesher and CI Fyfe arrived to the facility at 11:35 a.m. Therefore, the 4 year old child was in timeout for at least 13 minutes.
