Department staff were not permitted access to the facility as evidenced by CI Parsley knocked on the door multiple times before the provider's teenage son opened it, slammed the door shut, and could be heard yelling "Mom, someone's at the door!" CI Parsley waited 10 minutes on the porch, knocked multiple times again, but no further response was forthcoming. As he had done on 3 other occasions - 12/14/21, 12/17/21, and 1/7/22 - CI Parsley called both of the providers phone numbers and left voice mail messages, informing her of his presence. CI Parsley then left his business card with a note to call him on the porch as he had done previously.

The training shall be certified by a nationally-recognized organization, such as the American Red Cross, American Heart Association, or an equivalent certification, include an in-person skills assessment, and be approved by the department. At least one (1) caregiver with current certification in age-appropriate first aid and CPR must be on site at all times when children are present. First aid/CPR training may count toward the annual clock hour training requirement.

The parent or guardian of the child shall provide satisfactory evidence of the required immunizations.
