The facility's outdoor space was not safe for children's activities as evidenced by there are three spots where there is black weed protector underlining that is frayed, torn and accessible to infant and toddler children. The proivder has the option to remove the underlining from the playhouse, back section of the fence and the orange and green climber or the provider can cover the underlining with resilient surfacing.
