The facility's outdoor space was not safe for children's activities as evidenced by the preK playground had standing water (three feet wide) along the fence on the left side of the playground and standing water on the right side of the playground in the open grass space (two feet wide) though it had not rained in several days.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (6) (A) 4. states: The play area shall be safe for children's activities, well-maintained, free of hazards such as poisonous plants, broken glass, rocks or other debris and shall have good drainage.

Furniture/equipment, playground access door weather strips, in the preK classroom (ages 4-5 years) was not in good condition as evidenced by strips/seals were pulled away from the bottom of the door (over 6 inches) and the right side of the door frame (over 4 inches).
