Violation Requirements for infant/toddler sleeping equipment were not met as evidenced by a crib/playpen was not used for each infant in that a 2-month-old infant was sleeping on a changing table area as a bassinet at the top of a portable crib. Licensing Rule Reference 5 CSR 25-500.092 Furniture, Equipment and Materials (1) (B) 2. A. states: An individually assigned crib, portable crib or playpen shall be provided and used for each infant and toddler. Stack cribs shall not be used.
