Broken pieces of a metal wire that used to connect the fence to the bar were hanging down on child's eye level presenting a hazard to the children. .

Hazardous items as follows were accessible to children: bleach solution in a spray bottle , a plunger . The item(s) was/were located in a bathroom that is used by the children. The bleach bottle was located on the handle above the toilet, easy for children to reach. The plunger was in the bathtub. A provider agreed to keep the bottle out of children reach in a locked cabinet under the sink. A plunger was removed away from children's reach .
