The walls located in the small gym to to the left of two brown storage supply closets was/were not in good condition as evidenced by there was a 1 x 1 inch diameter hole in the wall. Children can easily put their hands in the hole and access the drywall behind the wall.

The facility's outdoor space was not safe for children's activities as evidenced by the lock used to prevent children from opening the gate and accessing the heating and air conditioning unit was not present on the gate. The lock was located on a higher part of the gate. Children can easily open the gate and have access to the unit.
