Outdoor equipment, brown and green climber, in the outdoor play yard was not free of sharp parts as evidenced by there were two broken pieces about 6 inches long that had sharp edges where a child could stick there hand in the whole and get cut/scratched.

Requirements for feeding equipment for infants and toddlers were not met as evidenced by inappropriate restraints were used in that a 9 month old child was sitting on a preschool chair at a table restrained by a rubber bendable tube holding her in place on the chair.

5 CSR 25-400.095 Furniture, Equipment and Materials 1 (C) 2 states: Infants and toddlers. Infants and toddlers who are unable to sit at a table shall be served meals at a feeding table, high chair, infant seat or other safely designed infant seating equipment. Equipment shall be provided which will allow a child to sit comfortably and securely while being fed. Appropriate restraints shall be used.
