Soft objects, loose bedding or any object that can increase risk of entrapment, suffocation or strangulation was observed as evidenced by 5 month old child observed sleeping on his stomach with a cloth bib attached to his neck. Caregiver in the room stated the child had been placed on his back to sleep and rolled over on his own and this was not the first time this child had rolled from his back to his stomach. There as also a pacifier in the crib with the child, along with what looked like an empty plastic medication syringe in the end of the pacifier, CI Cox was told this was to control the air flow. The bib was removed from the child's neck and the plastic medication syringe was removed from the crib during the inspection.

5 CSR 25-500.092 Furniture, Equipment and Materials (1) (B) 2. G states: Cribs, portable cribs and playpens shall be free of soft objects, loose bedding or any object that can increase the risk of entrapment, suffocation or strangulation. Examples include bumper pads, pillows, quilts, comforters, sleep positioning devices, sheepskins, blankets, flat sheets, cloth diapers, bibs, pillow-like toys, wedges, infant positioners, special mattresses, special sleep surfaces, and other similar items.
