The facility's outdoor space was not safe for children's activities as evidenced by multiple garden tools (including a hoe, a pitchfork, and a rake) were left on the sidewalk on the playground. A big branch from the walnut tree on the front playground was leaning all the way down on the play structure. There were mushrooms growing on the side playground .
