The facility's outdoor space was not safe for children's activities as evidenced by in the preschool playground for children ages 4-5 year's old, there is a 1/2-1 inch gap starting to grow in the resilient surfacing padding that is present throughout the entire playground. The gap in the resilient surfacing is a tripping hazard.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (6) (A) 4. states: The play area shall be safe for children's activities, well-maintained, free of hazards such as poisonous plants, broken glass, rocks or other debris and shall have good drainage.

The provider did not supply and serve food according to the Meal and Snack Food Chart as evidenced by CI Ray-Warnke and CIS Wheatley observed three children age 5 leaving the lunch table without a serving of fruit on their plate. Four children in the 5's classroom were observed throwing away their lunch at approximately 11:35am until 11:40am. CI Ray-Warnke visited all of the other classroom shortly after this observation. All other operating classroom had fruit present on children's plates at the time of service.
