Requirements for infant/toddler sleeping equipment were not met as evidenced by a crib/playpen was not provided for each infant in that the provider has closed and is not using 4 infant classrooms. All of the toys, shelves and equipment are still present in the facility, but all of the cribs have been removed except for 3 remaining cribs. The provider is going to submit a revision form to decrease capacity. Since CI Ray-Warnke was present for an anniversary inspection the provider needed to have 28 cribs available for children under 12 months. The provider is licensed for 28 children under 12 months. The provider is short 25 cribs. During the 9/28/2023 inspection CI and a director talked about the revision. When the revision goes through this violation will be removed. .

5 CSR 25-500.092 Furniture, Equipment and Materials (1) (B) 2. A. states: An individually assigned crib, portable crib or playpen shall be provided and used for each infant and toddler. Stack cribs shall not be used.

The requirements for obtaining 12 clock hours each calendar year were not met. For the calendar year of 2022, the staff listed need the following information: Latia Batts will need 1 hour, Brandy Rodriguez will need 1.5 hours and Ruth Zbylski needs 2.5 hours. During the inspection that was conducted on 9/28/2023 it was established that Brandy Rodriguez is the only person who is still missing the hours.

5 CSR 25-500.102 Personnel (3) (A) states: The center director, group child care home provider, all other caregivers, and those volunteers who are counted in staff/child ratios shall obtain at least twelve (12) clock hours of child-care related training during each calendar year. Clock hour training shall be approved by the department.
