Diapering requirements were not met as evidenced by the diapering table was not safe in that the counter top used for changing babies in the Pages room H had open edges on all sides, the changing pad was not fixed/fastened to counter top, and did not have a safety restraint, therefore a baby could roll/fall off and be injured.

The diapering table shall be located within or adjacent to the group space so the caregiver using the diapering table can maintain supervision of his/her group of children at all times.
