The requirements for protective outlet covers or twist-lock outlets were not met as evidenced by an electrical plate was broken in that the electrical plate was broken/cracked and barely hanging on to the outlet. This was in Voyager room on the west wall.

The item(s) was/were located The bleach water was located in Preschool B and the drywall crumbles were inside the furnace room door which would not close in the outdoor play area where the children had access to.
