Violation The provider did not follow required procedures for accidents, injuries and emergency medical care as evidenced by parents were not notified immediately when Child A fell on the playground and stated he hurt his shoulder and was crying. On February 29, 2024, CI Greer interviewed Parent A, Child E, Caregiver Keisha Black, and Caregiver Melanie Haniyeah. Parent A said Child A had tears in his eyes and said he was racing and tripped and fell on the playground when she picked him up on February 27, 2024. Child A said his shoulder and arm were hurting. Parent A took Child A to the hospital where it was determined that he had a broken collarbone. Child E said she remembers Child A falling on the playground. Caregiver Keisha Black said she remembers Child A falling on the playground on February 27, 2024. Caregiver Haniyeah said on February 27, 2024, Child A was running and fell down on the playground. She said he was crying and grabbing at his shoulder and saying he hurt his shoulder. She did not notify Parent A of the accident/injury. On March 5, 2024, CI Greer interviewed Child A and Child C. Child A said he fell down and pointed at his upper arm when asked where he was hurt. Child C said she saw Child A racing with other kids and fall. Licensing Rule Reference 5 CSR 25-500.192 Health Care (5) (A) states: In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions.
