1	3	INFANTS AND TODDLERS	Hailey Lister was the staff with the children in the right side I/T classroom. Children ranged in age from 13 months to 18 months. Children were in the preschool-I/T building.
2	15	MIXED	Kylee Crane and Rebecca Nichols were the staff with the children. Children ranged in age from 2 years to 3 years with 12 children being 2 years of age. Children were in the preschool building.

2	16	PRESCHOOL	Amanda Melloway and Haley Calvert were the staff with the children. Children ranged in age from 3 years to 4 years. Children were in the preschool building.
2	28	SCHOOL AGE	Hannah Bittle and Hope Crane were the staff with the children in the school age building. Children ranged in age from 5 years to 12 years. There was also a 14 year volunteer at the facility.

Violation Lighting requirements for rooms used by children were not met as evidenced by lighting was insufficient to observe and supervise children in that during nap time the lights were off in several classrooms and there was not a light source to provide enough light. Licensing Rule Reference 5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 4. states: Artificial or natural lighting shall supply at least ten (10) footcandles of light throughout each room used for child care.
