The facility's outdoor space was not safe for children's activities as evidenced by there were two hoses that were out and present in the walking patch towards the infant and toddler playground and the preschool playground. Hoses were loose and on the floor and able to be picked up and played with. Hoses can become a hazard if children pick them up, wrap them around themselves, wrap them around others. The hoses are also a tripping hazard for they were in the direction of travel towards both playgrounds.

Children could become injured on the rake if it is stepped on or the rake is picked up and swung around.
