The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by CI Duckworth observed a metal exhaust cover which was rusted and had sharp edges accessible to the children. Also, CI Duckworth observed 2 pieces of metal guttering in the play yard with sharp edges. CI Duckworth also observed a broken wooden edging piece that had a rusted rod through it. A lighter was on the grill on the deck. All these things were accessible to the children.

5 CSR 25-400.085 Physical Requirements of the Family Day Care Home (3) (A) 5. states: The play area shall be safe for children's activities, well-maintained, free of hazards such as poisonous plants, broken glass, rocks or other debris and shall have good drainage.

Hazardous items as follows were accessible to children: cleaning supplies, pest control items, and spray paint. The item(s) was/were located underneath the sink in the cabinet. Also the children had access to construction materials such as drills, saw, glue, box knife, etc. CI Duckworth observed these things on the counter in the kitchen. The construction items (drills, saw, glue, box knife, etc) were moved while CI was on site.

5 CSR 25-400.085 Physical Requirements of the Family Day Care Home (1) (J) states: All flammable liquids, matches, cleaning supplies, poisonous materials, medicines, alcoholic beverages, hazardous personal care items or other hazardous items shall be inaccessible to children.
