The floors located between the transition of the main classroom and hallway leading to the bathroom was/were not in good condition as evidenced by the provider has attempted to reseal the floor, the floor under the tape is cracked and missing the surfacing of the floor. If the tape is pushed on where the "fix" is the tape dips and chipping flooring is present under the tape.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 6. states: Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces.

Outdoor equipment, green and red large little tyke equipment, in the main playground was not in good condition as evidenced by the red slide had a crack that was present down the middle of the slide where children would sit on and slide down the slide.
