Children were not restricted to approved space as evidenced by the infants were being cared for in room B-7 and the toddlers were being cared for in room D-1. The room the infants are in is approved child care space, but is not approved for the care of children under 24 months of age. The room the toddlers are being cared for in is not approved for the care of any age child. During the re-inspection on 1-3-25 the infant/toddlers are being cared for in the same unapproved rooms as they were observed in during the inspection on 12-18-24.

The facility cares for children under the age of 24 months and at least one means of egress did not lead directly outside at level exit discharge as evidenced by during the time the HVAC is being replaced the infants and toddler are being cared for in B Hall and in D hall neither room has a means of egress that leads directly outside This was again observed during the re-inspection on 1-3-25.

5 CSR 25-500.087 Fire Safety (6) . states: Facilities caring for children under the age of twenty-four (24) months shall have two (2) means of egress out of each room that is dedicated for their use. One (1) means of egress shall lead directly outside at level exit discharge. Both exit doors shall be a minimum of thirty-six inches (36") wide or wider to accommodate the largest crib in the room. Doors shall swing in the direction of egress travel. The direct exit outside is not required if the entire facility is protected throughout by an approved supervised automatic sprinkler system.

Requirements for handwashing were not met as evidenced by during the re-inspection on 1-3-25 the infant/toddlers are still being care for in rooms without a sink that are hooked up to the facility's plumbing. The facility has added 2 portable sinks with cold running water and during the time portable sink was not working, an extra staff stepped into the room allowing the child that had their diaper changed and the staff doing the diapering to go to the hall bathroom to wash hands.

Caregiver Gracie Perryman was changing a diaper and when asked about handwashing she stated they are wearing gloves during diaper changes and using hand sanitizer in place of handwashing.
