The facility's outdoor space was not safe for children's activities as evidenced by there were three bricks located in the playground. One brick was located in the center of the playground. A third brick was found broken in the playground. A third brick was found near the fence of the playground with shredded pieces of the brick in the playground. The bricks are a tripping hazard for children. The bricks can be picked up and used dangerously or thrown. The pieces of the bricks that were ground or shredded can be put in children's mouths which would be hazardous to children if ingested.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (6) (A) 4. states: The play area shall be safe for children's activities, well-maintained, free of hazards such as poisonous plants, broken glass, rocks or other debris and shall have good drainage.

Outdoor equipment, Green and Black Truck, in the main playground was not free of sharp parts as evidenced by the back of the truck which is plastic was chipped and torn and presented sharp edges. Children can cut themselves on the sharp edge, children can also get their fingers caught in between the ripped plastic.

Outdoor equipment, Truck and Digger, in the main playground was not in good condition as evidenced by the top of the truck was located on one end of the playground, the digger was located on the other end of the playground and the broken truck was located in the center of the playground. The four wheels of the truck were found scattered throughout various parts of the playground.
