The facility's outdoor space was not safe for children's activities as evidenced by two red tops to plastic containers were on the ground of the playground, the containers were not present on the playground. The tops on the playground floor are tripping hazards for children whom run and walk on the playground.

The requirements for equipment to be clean, in good condition and accessible were not met as evidenced by there was a broken black and white toy bucket present on a toy shelf with tubes and marbles in it. The toy bucket was not in good condition, the front of the toy bucket was cracked, the bucket was plastic, the sharp plastic presented a sharp edge that could cut or injure children when picked up.
