3 5 INFANTS AND TODDLERS Bridge McGrew, Dani Crowder and Malia McCarty were the staff with the children. Children ranged in age from 1 year to 2 years.
3 6 INFANTS AND TODDLERS Lily Deroy, Ashley Haddix and Alexa Green were the staff with the children. Children ranged in age from 3 months to 2 years.
3 10 MIXED Courtney Monshower, Ginney Ison and Tammy Jackson were the staff with the children. Children ranged in age from 3 to 5 years.

The walls located in the following classrooms: EHS 3, EHS 2 and EHS 1 was/were not in good condition as evidenced by there was chipping paint in EHS 3 and EHS 2 in multiple areas along the walls and in EHS 1 there were small holes in the wall along the right side.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 6. states: Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces.
