The walls located on the west green wall in Toddler 2 and on the purple south wall in the lunch area of Toddler 1 was/were not in good condition as evidenced by there were holes in the drywall.

Diapering requirements were not met as evidenced by the diapering table was not located where caregiver could maintain supervision of his/her group of children. The diapering table was located in the preschool room against the wall where staff is facing the wall when changing diapers and do not have supervision behind them for the children that may playing behind the staff.

The diapering table shall be located within or adjacent to the group space so the caregiver using the diapering table can maintain supervision of his/her group of children at all times.

The facility's outdoor space was not safe for children's activities as evidenced by tall grass and standing water as the play yard doesn't have good drainage. Kirksville just got a bad storm. Water standing in lower areas of play yard that does not have good drainage. Play yard needs mowed.
