The ceilings located throughout the building was/were not in good condition as evidenced by 22 ceiling tiles (13 tiles in the right side preschool classroom, 3 tiles in the left side 2-3-year-old classroom, 3 tiles in the first hall bathroom, and 3 tiles in the second hall bathroom) showed evidence of water damage, including brown water stains. One ceiling tile in the kitchen was falling down and not fully attached..

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 6. states: Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces.

The walls located at the entrance of the 2- to 3-year-old classroom was/were not in good condition as evidenced by the gate door that allows access to the 2- to 3-year-old classroom is off the hinge at the base of the gate. The gate door falls off the hinge when opened and the door is scraping along the floor.
