3 7 INFANTS AND TODDLERS In the Infant classroom Yu McMullen, Sherry Corpening and Basil Isle-Colon were the staff with the children. Children ranged in age from 5 months to 9 months.
3 6 INFANTS AND TODDLERS Toddlers classroom was on the I/T playground. Kaylee Parker, Jill Farris and Hattie Robinson were the staff with the children. Children ranged in age from 14 months to 23 months.
3 11 MIXED Classrooms Pre-Primary North and Pre-Primary South were on the pre-primary playground. Brianna Roller, Abbey McCullough and Haley Byrd were the staff with the children.
7 0 NO CHILDREN IN ATTENDANCE Additional Staff: Melaina Drissell (director) Sheila Dalton (office) Sarah Larrabee (Kitchen) Bridget Weeks (on break) DeMarcus Jackson (on break) Sarah Voyles (new hire doing training) and Danielle Ammon (new hire doing training) were all present at the facility during the inspection.
5 23 PRESCHOOL Classrooms Primary East and Primary West were on the Preschool Playground. Tim Lothian, Yunalsy Gallardo, Jett Cline, Alimah Moad and Symonne Sparks were the staff with the children. Children ranged in age from 3 years to 5 years.

5 CSR 25-500.122 Medical Examination Reports (1) (B) . states: Medical examination reports shall include either a Tuberculosis (TB) Risk Assessment form, completed and signed by a health care professional, or a negative tuberculin skin test (TST) completed not more than twelve (12) months before beginning work in the facility. The Tuberculosis (TB) Risk Assessment form, revised March 2014, is incorporated by reference in this rule, as published by the Missouri Department of Health and Senior Services, PO Box 570, Jefferson City, MO 65102 and available by the Missouri Department of Health and Senior Services at https://health.mo.gov/living/healthcondiseases/communicable/tuberculosis/tbmanual/pdf/RiskAssessmentform.pdf. If the person has signs or symptoms of tuberculosis, or risk factors for tuberculosis, then testing for tuberculosis shall occur.
