2 8 INFANTS AND TODDLERS In the Infant/Toddler class on the left side, Victoria Levitt and Miranda Adkins were the staff with the children. Children ranged in age from 17 months to 23 months.
1 3 INFANTS AND TODDLERS In the Infant/Toddler class on the right side, Hailey Lister was the staff with the children. Children ranged in age from 4 months to 17 months.
2 13 MIXED In the Toddlers class, Stephanie Chishom and Tosha Wimer were the staff with the children. Children ranged in age from 2 years to 3 years with 12 children being 2 years of age.
2 0 NO CHILDREN IN ATTENDANCE Director, Holly Crane and staff member Shana Rounsavall, kitchen, were both present during the inspection.
2 18 PRESCHOOL In the Purple class, Rebecca Nichols and Kylee Crane were the staff with the children on the playground. The children ranged in age from 4 to 5 years.
2 13 PRESCHOOL In the Gold class, Amanda Mellaway and Erica Crowe were the staff with the children. Children ranged in age from 3 years to 4 years.

The walls located in the purple classroom and in the toddler classroom was/were not in good condition as evidenced by in the purple classroom under the front corner window there was chipping paint and in the toddlers classroom behind the bathroom door there was peeling paint where a door stop had come off.

5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 6. states: Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces.

Licensing Rule Reference
5 CSR 25-500.192 Health Care (4) (A) states: No child shall be permitted to enroll in or attend any day care facility caring for ten (10) or more children unless the child has been immunized adequately against vaccine preventable childhood illnesses specified by the department in accordance with recommendations of the Immunization Practices Advisory Committee (ACIP). The parent or guardian of the child shall provide satisfactory evidence of the required immunizations. Satisfactory evidence means a statement, certificate or record from a physician or other recognized health facility or personnel, stating that the required immunizations have been given to the child and verifying the type of vaccine and the month, day and year of administration.
