The premises were not safe and suitable as evidenced by at approximately 12:35 p.m., CI Duckworth observed Thelma Barker and Kayla Zesch carrying a 1 year old child asleep on a cot from the Infant Room to Toddler 1. CI Duckworth observed each staff member carrying one end of the cot about 3-4 foot off the ground while the child remained asleep on the cot. This transfer of the child was not safe.
