The facility's outdoor space was not safe for children's activities as evidenced by there was black tarps showing from underneath the mulch that children could get their feet caught under an is a tripping hazard. CI Duckworth tested the tarps showing and was able to get her foot underneath the resulting in the example of a tripping hazard. There were approximately 4 places in the outdoor play yard around the trike track and in the middle of the track.
