The walls located in the boys and girls bathroom located downstairs, in the upstairs classroom, in the upstairs eating area was/were not in good condition as evidenced by there was peeling paint on the walls behind the toilets in each of the boys and girls bathroom, in the upstairs classroom on the peach wall by the back exit door there was patch of peeling paint and in the upstairs eating area under the window facing the parking lot there was a crack with peeling paint.

The facility's outdoor space was not safe for children's activities as evidenced by there were multiple spots where the weed fabric has come above the pea gravel creating a tripping hazard for the children. Specifically the area located right outside of the school age door and an area between the concrete track and climber with yellow slide.
