Infants and toddlers. Infants and toddlers who are unable to sit at a table shall have one (1) piece of mealtime feeding equipment for every four (4) infant/toddlers which may include feeding tables, high chairs, infant seats or other safely designed infant seating equipment. Equipment shall be provided which will allow the child to sit comfortably and securely while being fed. Appropriate restraints shall be used.

Compliance Inspector (CI) Jessica Glover conducted an investigation regarding the allegation that Child A (13-months-old) had fallen out of a highchair. After conducting the investigation, CI Glover finds this allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:

Infants and toddlers who are unable to sit at a table shall have one (1) piece of mealtime feeding equipment for every four (4) infant/toddlers which may include feeding tables, high chairs, infant seats or other safely designed infant seating equipment. Equipment shall be provided which will allow the child to sit comfortably and securely while being fed. Appropriate restraints shall be used."
On February 27, 2023, CI Glover interviewed Assistant Director Rekeshia Simmons who stated that staff utilized the highchairs without strapping the children in. Ms. Simmons stated that many of the straps were thrown away due to them getting dirty, but that new ones were ordered after Child A had stood up and fallen out of the highchair on February 8, 2023. Even though Director Jessica Tran stated that she told Parent A (Parent of Child A) that straps would be used with the children in the highchairs moving forward, CI Glover conducted a walkthrough of the facility and observed two children in highchairs without being strapped in. CI Glover observed one child in a highchair that did not have any straps while another child was sitting in a highchair with straps but was not secured.

The facility shall notify all staff members of the violations which were substantiated, and specify the actions to be taken by all staff in order to comply with all licensing violations cited. A copy of the memo, letter, or staff meeting agenda(s) shall be submitted to the Office of Childhood.
