Compliance Inspector (CI) JD Greer conducted an unannounced investigation of the allegation that Parent A (Parent of Child A) was concerned the facility may be out of ratio. The allegation is substantiated based on the following information:
5 CSR 25-500.112(1)(A) which states, "Birth Through Two (2) Years. Groups composed of mixed ages through two (2) years shall have no less than one (1) adult to four (4) children, with no more than eight (8) children in a group;"
On April 18, 2023, CI Greer conducted an unannounced walkthrough of the facility and observed Caregiver Dani Holst was present with five children, 4 months old to 6 months old.
On April 18, 2023, CI Greer interviewed Director Staci Craigmile who stated Caregiver Dani Holst was out of ratio because Caregiver McKayla Morris had just been thrown up on by a child. Instead of calling for assistance Caregiver Morris passed the child over the half wall to Caregiver Holst while she changed her shirt.

5 CSR 25-500.102(1)(B) which states, "Child care personnel shall cooperate with the department."
On April 20, 2023, CI Greer attempted to gain a copy of staff and child attendance records from Director Staci Craigmile. The records were never received.
On April 27, 2023, CI Greer attempted to gain a copy of staff and child attendance records from Director Staci Craigmile. The records were never received.
On May 5, 2023, CI Greer attempted to gain a copy of staff and child attendance records from Director Staci Craigmile. The records were never received.
