Compliance Inspector (CI) JD Greer conducted an unannounced investigation of the allegation that Child A (21 months old) was pinched and bruised when being buckled into a high chair. On April 26, 2023, CI Greer interviewed Assistant Director Shayna Lam who stated on March 1, 2023, Child A was accidentally pinched by the highchair buckle. Parent A was not notified until the end of the day.
On April 26, 2023, CI Greer interviewed Caregiver Annie Lucero who stated on March 1, 2023, she was buckling Child A into a highchair. The buckle accidentally pinched Child A. Caregiver Lucero completed an incident report but forgot to tell Parent A until the end of the day.
On April 26, 2023, CI Greer reviewed an incident report for Child A from March 1, 2023, and observed the report is not signed by the parent. The report does not state when or how Parent A was notified.
